"Dear Jesus,
I know I'm a sinner, and I ask for your forgiveness.
I believe you Jesus as my Savior and that you died for my sins on the
Cross at Mount Calvary.
I believe God, the Father raised you from the dead to life on the third day.
I trust you as my Savior and want my spirit redeemed through you.
 I will follow you Jesus this day forward.
And I will repent of my sins daily.
 I will have you as my guide,
Please help me to do your will.
I pray this in your name my Lord Jesus Christ.
Amen."
