Forget me not please.
Will you forget me?
Will you remember?

Remember the way I died.
How I took my life.
Will you forget me?

Im so sorry.
But I didnt know what else to do.
I did the only thing I knew I could do.
I took my life.

Sorry to cuase you pain.
Sorry to hurt you.
Im very sorry.

Please forget not the goodtimes.
But forget the bad.
Forget how I died.
Forgive my suicide

I like the play on words with the thought of losing the memory of someone and them wanting to be remembered with a death suicide note. But if converted into a poem you would think that you would play with the form because some is dieing or about to die and their thought pattern is not in the right order. Something to think about. The rest of the poem needs some minor line breaks with commas or paragraph break. I think the tone is lost at the end but could be redone to your satisfaction.
